Dear Parents/Guardians:
Your child will be bringing home an emergency form next week. This form is for the 2024-25 school year. Please complete the form and return it to school by May 24, 2024. Fifth grade students will also be bringing home physical forms for upcoming 6th grade. Please disregard this physical form if you previously turned in a form during your child’s fifth grade year.
